Smooth Care Transitions
Patients are particularly vulnerable as they transition from hospitals to their home or post-acute care settings. Older patients with complex health issues are at the highest risk of complications. We bring 40 years of experience in care transition to help avoid this. We are committed to ensuring smooth care transitions and care coordination, which helps reduce avoidable adverse events.

Coverage Where it Matters Most
Our service area covers a 50-mile radius of Hutchinson, Ks. This means we provide the full range of home health or hospice services to you or your family member at their home or home-like setting. This includes the set up and delivery of any necessary medical equipment and supplies.

Patient-Centered Healthcare with Family Support
We believe in providing support to the patient and the family. The patient is supported by a team of healthcare professionals. The family is also supported. Social workers provide social support and can help connect the patient and the family to resources. Chaplain services provide emotional and spiritual support. 

Personalized Care & Specialty Programs
Working closely with you and your physician, we develop a plan of care that is tailored specifically to your needs, wants, and goals. Care Plans can include rehabilitative services, nursing, wound care, home health services such as bathing, and homemaker services such as meal preparation. Complex conditions are supported through an interdisciplinary approach. Care teams are in-touch with patients frequently to amend one’s plan of care to achieve the best results. Customized care plans improve quality of life and achieve better outcomes, even for complex conditions.
