Questions to Ask Your Doctor Prior to Requesting an Estimate

What is the exact name of the procedure(s)?
Ask the doctor to clearly print the name of the procedure. Correct spelling is important as many surgery names sound similar.

What is the CPT code for the procedure(s)?
Ask the doctor for this information. CPT codes are the billing codes that are used by providers usually for physician services. If you do not have this please be sure to have the exact name of the procedure.

What ICD-10 diagnosis code(s) will be used?
Ask the doctor for this information. The International Classification of Diseases codes identify your health condition and allows healthcare professionals anywhere in the country to have a shared understanding of conditions.

What is the doctor’s name that ordered the procedure(s)?
Be sure to have the doctor’s first and last name. For surgical procedures please be sure to provide the surgeon’s name.

Will you be staying overnight?
Ask the doctor if you will be admitted to the hospital as an inpatient overnight or if you are expected to be treated on an outpatient basis.
