What to Bring

Be sure to have the following items with you for your first visit:

• Medication List
• Guardianship Paperwork (if applicable)
• Insurance Card or one of the following:
  • Proof of household income (Federal Income Tax Return, W-2, and three current pay stubs)
• Current benefit determination letter/application letter for disability, workers compensation, unemployment, general assistance, or food stamps
