Social Security Outreach, Access and Recovery (SOAR) is a Horizons program that helps homeless individuals – or those at risk – who also suffer from mental illness and/or addiction.
SOAR connects eligible adults to Social Security/Disability Insurance (SSI/SSDI), with the goal of meeting basic and mental health needs, and stabilizing the individual.
Then, we work closely with the Department of Children and Families and Vocational Rehabilitation to gain employment and utilize the Working Healthy Program.
Qualifying adults are those at risk of homelessness and who have a serious mental illness or impairment and a co-occurring substance use disorder.
